I will hazard a guess that that barrel is a vacuum secondary which will not open until the engine is under some kind of load and there is little to no vacuum in the manifold. To see that barrel open you will most likely have to have the throttle wide open not just some higher rpm in neutral.
Of course you may already be on top of all that and it may have a buggered diaphram in the vacuum thingy which should control that barrel.
