Righto.. first, undo the two little bolts that hold the housing together, spin the lower part (smaller part) around 180' and bolt back together. This will give you a better angle as it will now point slightly forward instead of backward (see in my pics in post #3 how the lower pipe angles forward, not backwards). If you're mounting it like mine, this will now point down and forward behind the engine mount, so you can run a pipe under the mount and forward to the lower rad.
Looking at the top section, the big pipe pointing forward goes to the water pump.
The pipe above that goes to the front of the head where the top rad hose enters the head.
The back pipe (side pipe in your case) goes to the back of the head. Your heater core will need to be Tee'd into this line. (two Tees, one in, one out via the heater tap)
I'm not running a heater so there are no Tee's in my rear waterline in the pic above.
DON'T run the water direct from the back of the head, to the heater core, then to the thermo housing! If there is no bypass of the heater core this will cut the water flow from the back of the head whenever you turn the heater to cold, as that closes the heater tap, and cooks your motor. So make sure your setup allows free flow at all times.
