MX83 Stock Auto Probs???

Hi, I'm am having a problem with the auto trans. it's starting off in second, I can only drive off in first if I manually select it.
Also the torque converter is not locking and the O/D light on dash is flashing while I am driving, any ideas.
Also I am looking for information on A340E transmission fault codes and how to check them.
