is it something to do with the timing chain? like its slapping the inside of the cover?. were the guides and tensioner replaced?

can you get it to knock constantly enough to pull plug leads one by one and isolate it to a cylinder?

if the timing marks line up its gotta be right.
The cam pulley bolt could be loose, but i doubt it would cause a rattle, compression test?

I had a Hilux which at 502 000 klms developed the same problem. After driving it like an old woman and not driving it hard i managed to get a couple more thousand k,s before it near craped itself on the way home from work one day.
After extracting the motor and pulling the sump off, revealing a flogged out no3 big end.
Fortunatley there was no damage to the crank or caps, so i purchase a set of king bearings and reassembled and of she went sweet. Probably for another 500 000 or so, but i will never know as sold it soon after.
Just to let you know that when i started it up cold it ran smooth and quiet, however upon warming up it developed a knocking noise which did sound like from the top ( harmonics at a guess ). At first it was intermittent and at mid range and as i kept driving it after living with it, it progressively got worse. As these things do when no attention is paid.
I hope for your sake it isn't and that it is an easy fix, which for most the time it is. The worst thing is sourcing the problem. Mind you the bigend bearings in my case was an easy fix but getting the motor out and back in, well that sucked 10 fold.
    Hope i have helped.
    Cheers Simon

You can do big ends with the motor in car but i wouldnt recomend it lol, with bearing failure you going to have metal shavings throughout the motor so a full strip down is the go.
If you can get another motor thats good, do that for sure!
