I just thought that I'd add to this as I've had the same problem in the last couple of weeks, a knock that sounds when I go over bumps or on steering.

It turned out to be the big nut holding the damper in the strut (not the one at the centre of the strut top but the one under the bump stop at the centre of the spring).
I replaced this with the propper one as it had one with too coarse a thread on it and the insert was pretty much rattling around in the tube because it wouldn't screw in far enough.

    I did this recently.

I went to a metal-pipe selling place, took one of my inserts in, and asked to buy some of the thickest-wall piping they had less than or equal to the diameter of the bottom of the insert. I ended up with some 3mm wall steel stuff that happend to be exactly the same width as the inserts (43.5mm I think), and they cut it exactly to length. 4 peices (I got 2 45mm and 2x 55mm to play with) cost me 5 bucks.

^^ about the same, I got a fitter at work to make me some spacers out of steel for the bottom then used some large washers for the top in order to place the insert in the tube where I wanted it.

Another thing I've seen used is a pipe joiner, (2 threaded ends with a hex nut in the middle). they cost about $3-4 ea from bunnings in aluminium and the hex can be found in just the right size to fit in the tube.

For the top, you can have the same type of tubing lathed at each end such that it sits sungky & securely on top of the insert and inside the top of the [big threaded part that keeps strut insert inside strut that I don't know the name of]. This is what we ended up doing.
