In the defence of whoever you guys paid to fix it, it's a really tenacious piece of malware. From what I've read it has a few ways of re-establishing itself if you don't do a really thorough job of removing it.
Still, it's a shame it wasn't properly tested after being "fixed"

Ok easy fix for that... the default changed after version 4.2.0, just a setting to put it back to normal.

I went through the site this morning and found a couple of sus entries in the log files, I took measures to block access to the scripts I suspect they were using to implant the malware (which I found and removed), as well as upgraded a few other bits and pieces.
The person who thought that executing php code from the database was a good idea for plugins should be hung and quartered.
