    Default Brake sleeving issues

hello all. heres my problem. i had to have sleeves made up for my brake setup and whilst racing it the sleeve that goes over the base of the spindal has come loose and we found it in the hub.

Q being what would cause something that tight on th spindal to come loose? we have put it back on but im fairly sure it will come loose again.

so whats peoples thoughts on keeping it in place.

    cheers mat

I came up with an idea Matt.
Drill a 2mm hole through the sleeve and about 1mm deep into spindle, then place a ball bearing in it.
When the wheel bearing is in place it'll stop the ball from dropping out and the ball will stop the sleeve sliding off.
