Toymods PDF membership application form

I converted the available membership form (available as a pdf from here) into an editable pdf form such that you can enter the required details and then print it. (available here).

I'm sure the form doesn't get used that much, but if you guys have the time to replace the standard / current form with the one I have made editable it may make life easier for someone in the future

I also made a 2006 version, it probably makes more sense just to update with the 2006 version in the new year (available here)

That will be very useful id say. I would have used it when I sent in my registration information as my hand writing leaves allot to be desired.
